เนื่องจากสินค้ารุ่นนี้ยังไม่มีระบบ QR Code หรือ Serial Number สำหรับลงทะเบียน Cool Reward ทางเราจึงเปิดฟอร์มพิเศษนี้เพื่อให้ลูกค้าสามารถลงทะเบียนรับ คะแนน Cool Reward ได้ตามสิทธิ์

เงื่อนไขการรับคะแนน Cool Reward: - ผู้มีสิทธิ์รับคะแนนจะต้องเป็นสมาชิกของ Cool Reward โดยลงทะเบียนผ่าน LINE Official Account (LINE OA) และเลือกบัญชีที่ลงทะเบียนไว้ในช่องทางที่กำหนดในฟอร์มนี้

- คะแนนจะถูกโอนเข้าบัญชีของสมาชิกที่ผ่านการตรวจสอบภายใน 15–20 วันทำการ นับจากวันที่ส่งแบบฟอร์มและหลักฐานครบถ้วน

- ทางบริษัทขอสงวนสิทธิ์ในการพิจารณาและตรวจสอบความถูกต้องของข้อมูลและหลักฐานที่ส่งมา

- หากข้อมูลไม่ครบถ้วนหรือไม่สามารถยืนยันตัวตนได้ ระบบจะไม่สามารถทำการมอบคะแนนให้ได้

As this product did not come with a QR Code or Serial Number for Cool Reward registration, we have created this special form to allow customers to register and receive their eligible Cool Reward points Cool Reward Point Issuance Conditions: - Eligible customers must be registered members of Cool Reward via the LINE Official Account (LINE OA) and must select their registered account through the designated section in this form.
- Points will be credited to verified accounts within 15–20 business days after complete form submission and documentation.
- The company reserves the right to review and verify all submitted information and evidence.
- If the submitted information is incomplete or identity cannot be verified, the system will be unable to process and issue the points.
